 Prenatal Care Coordination (PNCC)pregnant

Prenatal care coordination helps eligible women have the healthiest baby possible. Public health nurses provide education, support and connection to other community resources through conducting regular home visits throughout the pregnancy. By educating expectant mothers on the importance of medical prenatal visits, the harm of alcohol and smoking on an unborn baby, infant development and proper prenatal nutrition, the chance of a healthy new baby increases.

Newborn Visits

newborn visit

First time parents of newborns will be contacted to offer a home visit with a Public Health Nurse. The Nurse will assess the newborn as well as provide information, support, and access to continuing care and resources for families with newborns
